The basic cause of hair loss in humans is genetical tendency. (Other factors: severe malnutritions, hormone imbalances, autoimmune diseases, infections, burns, stress, etc.)
. Male type hair loss, called androgenetic alopesia in the medical language, is classified according to the Norwood Scale. The enzyme called 5-alpha reductase converts the male hormone testosterone into a product called dihydrotestosterone (DHT), and DHT is excreted through urine and skin. DHT, which is believed to cause hair loss in people with genetic predisposition, weakens the structure of the hair follicle and causes its permanent loss.
. Hair transplantation surgery (with FUE/ Follicle Unit Extraction technique) is the process of taking the hair follicles one by one from the donor area and opening holes on the area needing transplantation and placing them in this holes one by one.
. The aim of the surgery is to obtain a better image from the current situation by transplanting as many follicles as possible (optimal number) to the regions where the hair is missing or falling away. The outcome depends not only on the success of the surgical team and procedure, but also on the quality of the hair follicles, the thickness of the hair follicles, the shape, color, amount of hair loss, blood circulation in the transplantation area, general health of the patient, diet and care.
. The operation is performed under local anesthesia.
. The duration of the operation is 3-8 hours, depending on the size of the transplantation area.
. The follicles transplanted 1-2 mm in length will fall down in the first month after the operation. The transplanted hair starts to grow up after 2-3 months and after about 6-7 months it grows about 70 % and the hair growth cycle is completed in about 12 months. The hair growth in the vertex region is slow and is completed in about 18 months. It is not possible to examine objectively the result of the operation until the process is complete. The transplanted hair adapts to the area where they are transplanted in 24 months and reach their regional thickness.
. Tobacco products can cause hair loss since they affect blood circulation. Thus, cigarette, hookah, etc. must be avoided 1 week before the surgery as well as 1 week after the surgery.
. 3 % of the patients may not have the results expected from the surgery without any given reason. In other words, if the procedure is performed properly, hair transplantation is most likely to give a good result. However, as in other operations performed on the human body, hair transplantation surgery always holds a small risk, and the outcome cannot be guaranteed in advance.
• When it comes to women considering hair transplantation (remembering telogen effluvium), some bloodwork must be done before the surgery.
. Hair loss is a lifelong process. For this reason, it is recommended that you use a variety of medications to support the result of the hair transplantation and reduce hair loss immediately after the operation. If a thicker hair is desired, thickening operations may be required. In other words, to achieve better results several operations may be required.
Complications and risks of the operation
• Shock hair loss: In rare cases undesirable hair loss may occur in the forms of patchy (due to hypoxia/ reduced oxygen transport), in the donor area and/ or transplant area. This situation is temporary. After 4-5 months on average, lost hair grows completely.
• There is a risk of infection as in every operation. During the period of hair growth some pimples may occur on the scalp. This condition may require treatment. In addition, pain (a stretch or a feeling of pinprick), swelling, numbness, allergic reaction, redness, itching, necrosis, etc. may occur.
Before The Surgery
1. Hair transplantation is not suitable for some types of hair loss. For this reason, it is necessary to seek medical advice from a physician before the surgery.
2. If you have any illness or any allergic condition, or if you use any medicine inform your physician before the hair transplantation.
3. If you are using aspirin, plavix, etc. drugs ("blood thinners") that prolong the clotting/bleeding time, stop them 1 week before the operation with the knowledge and written approval of the expert (cardiologist, internal medicine specialist etc). Please continue to use your other medications.
4. Blood work is needed to determine if you are suitable to have the operation.
The Day Of Operation
1. Wear a shirt that can be buttoned up from the front.
2. Before surgery, your doctor will evaluate you, examine you and draw a line on your scalp to define the transplantation area border.
3.You are dressed in a patient scrub and your hair is shaved 1 mm long (partial shaving is done in "unshaven transplant").
4. You'll lie face down and local anesthesia is applied to the area on the scalp from which the hair follicles are going to be taken. The follicles are taken, sorted, counted, and classified. The follicles removed are preserved in serum in glass petri dishes which placed on cool-packs.
5. Local anesthesia is performed also on the area where hair is to be transplanted. The holes in which the follicles are going to be transplanted are opened depending on the direction of the hair, the angle and the planned density.
6. A serum containing preservative medications is given to the patient. If the serum cannot be given, the medicines are given orally.
7. Hair roots are placed in the opened holes. The area where the follicles are taken is dressed. A tennis headband is placed on it.
8. The patient is discharged. He is informed about the issues which he will need to pay attention to. He is guided on how he is going to be taking his medications.
9. Start applying cold compress to the forehead and the temporal region for 10 minutes every 1 hour on the same day.
10. The patient should sleep on a high pillow avoiding any contact to the transplantation area.
After the Operation
Day 1: using antibiotics, painkiller, edema/ swelling soothing drug, cold compress application, sleeping on a high pillow
Day 2: using antibiotics, painkiller, edema/ swelling soothing drug, cold compress application, sleeping on a high pillow and the first wash
Day 3: using antibiotics, painkiller, edema/ swelling soothing drug, cold compress application, sleeping on a high pillow and the second wash
Day 4: using antibiotics, painkiller, edema/ swelling soothing drug, cold compress application, sleeping on a high pillow and the third wash, removing the tennis headband
Day 5: using antibiotics, fourth wash
Day 6: Fifth wash
Day 7:sixth wash
Day 8: seventh wash
Day 9: eighth wash.
Day 10: ninth wash.
Day 11: Tenth wash. Scabs are removed with a hard pressure massaging.
• Personal care is required for 10 days following hair transplantation and it is recommended that you spend this time in a healthy (more hygienic) environment, like as your home. During this time, there may be edema-swelling at different parts of your face and eyes and there may be crusting on the area where the hair follicles are taken and on the area where the transplantation is performed.
• The wound healing process continues 7-11 days. Swelling of the forehead may occur from the 2nd to the 5th day. Rarely, it can take up to 10 days.
• The numbness may continue for several months on the are where the follicles are taken and on the transplantation area.
• All or a part of the transplanted hair may not grow. Average 5 % loss of transplanted hair may occur. Accordingly, some marks can be seen on the transplant area, but this cannot be seen by the naked eye.
• To reduce the likelihood of problems, the prescribed antibiotics, painkillers and similar drugs must be used without interruption, dressings and bandages should be regularly applied as recommended.
• If there is any problem related to the surgery, contact the Physician (who performed the operation), because he/ she is the person who will be the best to evaluate the source of the problem and find the solution to the problem.
• After the hair transplantation, there will be permanent small spots like as shadow on the region where the follicles are taken. The appearance of these spots changes from one individual to another. For this reason, the shaving in the donor area should be performed according to the surgery process. The donor area permits a minimum of 4-10 mm of shaving after the surgery.
. Do not smoke or drink alcohol for 7 days after the operation.
• Please apply the cold compress for 4 days.Put cold compress on your forehead and temporal area for 10 minutes every hour. Do not apply it on your eyes or the transplant area.
• It is important for you to keep the black bandage for the first 4 days. If swelling happens please remember that it will not effect the final result.
• Please sleep on high pillow the first 4 nights. It is better that you keep your head a little bit high for the first 4 nights. You can sleep backwards or carefully on both sides.
• Do not overbend and make sex the first week.
• Please do not make any heavy exercises and sports, do not hold heavy subjects, do not stay under sun, do not use steam bath (sauna, Turkish hammam), do not go solarium, do not use helmet or hard hat for 2 weeks.
• Do not use shaving machine for 3 months, after 15 days you can cut only using scissors. You can dye your hair after 2 months.
Washing
• Recipient area should be washed very gentile for 9 days, without massage.
• First apply Pan(to)thenol foam sprey on the recipient and donor areas. Wait for 15-20 minutes and rinse off it with warm water with light touches without pressure.
• Foam the shampoo in your hand and apply it on recipient area without massage. Donor area can be washed by making light massage.
• In the 10th wash apply hard massage to finish all scabs (which depends on the direction of the hair).
Last control is made after 10th wash.
